What is HOM?
People with developmental disablities are
carefully matched by HOM with families who open their home and hearts to
provide enduring relationships.
HOM insures quality of service by carefully matching,
approving, training, monitoring and supporting the Host Families.
Who Are Host Families?
Host Families are single parents, individuals,
traditional nuclear families, multi-generational families, or any family
demonstrating stability, warmth and the capacity to include another
family member.
Who are the Residents?
People, 18 years of age or older who are
determined to have a developmental disability such as mental
retardation, autism or cerebral palsy.
Family Size
In keeping with the idea that small family units
provide more person centered supports and choices, the emphasis of
placement will be upon placing one Resident per Host Family.

Family living as a residential option is founded
on the premise that enduring relationships are crucial to a person's
growth, development and wellbeing.
The family is the foundation of society -- where
people develop self-esteem, learn to exist in interdependence with
others, and master life skills.
Families allow for participation and inclusion in
everyday life. (i.e., community involvement, trips, vacations)
The family living option provides for a maximum of
person-centered services and supports and is an ideal living and
learning option for the Residents we serve.

Interested in Becoming a Host Family?
Submit an application to HOM
Undergo an asset procedure which includes, but not
limited to, a home safety inspection, Credit check, and a criminal
record review.
Obtain a Certificate of Approval from HOM
Agree to adhere to the policies, procedures, rules
and regualtions of HOM
